Sub-Saharan Africa Still Carries Most of the World's HIV Burden, Though the Numbers Have Shifted

For decades, sub-Saharan Africa has carried a share of the world’s HIV and AIDS burden wildly disproportionate to its share of the world’s population. Around 2011, UNAIDS estimated the region accounted for roughly 69 percent of people living with HIV globally and about 70 percent of AIDS deaths that year, figures that meant a region home to well under 15 percent of the world’s population was absorbing more than two-thirds of a global epidemic’s human cost.

That concentration has eased somewhat in the years since, though it remains severe by any global comparison. Of the roughly 40 million people estimated to be living with HIV worldwide today, close to 64 percent, around 25.8 million people, are in sub-Saharan Africa, a modest decline in the region’s global share from the 2011 figures but still a level of concentration no other world region comes close to matching. Worldwide adult HIV prevalence sits at roughly 1.2 percent; in sub-Saharan Africa specifically, it runs at about 9 percent, a nine-fold difference that reflects both the epidemic’s specific transmission dynamics in the region and the historical timeline of when antiretroviral treatment and prevention infrastructure actually reached different parts of the world.

The regional average obscures enormous country-to-country variation. Adult prevalence rates run below 0.1 percent in Somalia, Comoros, and Sudan, essentially comparable to low-prevalence countries elsewhere in the world, while at the other extreme, Eswatini’s adult prevalence stands at roughly 25.9 percent, Lesotho’s at about 19.3 percent, and South Africa’s, the country with the largest total number of people living with HIV anywhere in the world, at roughly 17.8 percent. That spread means “sub-Saharan Africa” functions more as a geographic grouping convenient for global reporting than as a single epidemiological reality, with southern African countries specifically carrying a burden that dwarfs the rates found in much of West or East Africa.

The longer trend line, obscured by any single year’s snapshot, points toward real and sustained progress. Researchers tracking the epidemic estimate that an individual’s lifetime risk of acquiring HIV in sub-Saharan Africa has fallen by roughly 60 percent since the epidemic’s peak in 1995, a decline driven by expanded testing, the scaled rollout of antiretroviral therapy that both treats existing infections and reduces transmission risk, and sustained prevention campaigns across the region’s hardest-hit countries. The region’s disproportionate share of the global burden hasn’t disappeared, and UNAIDS’s own 2030 targets for further reduction remain unmet in several countries, but the trajectory over the three decades since the mid-1990s peak has moved consistently in one direction, even as the raw regional share, still the majority of the global epidemic by any measure, remains a defining fact of global public health.
